EVALUATION PROCESS AT A GLANCE

1. Orientation:

  • Read everything under overview, which is located in the menu on the left.

  • Attend a seminar or order the DVD on the Neurodevelopmental Approach from our website.

2. History Form:
Mail the completed history form, along with a photograph of your child to the address at the top of the form. You do not have to complete the 7-day Food Diary (mentioned on page three) before submitting the History Form; you can bring it with you to the appointment. If time is limited, email the History Form to support@littlegiantsteps.com or fax the form to 972-325-4119. Any additional testing can be reviewed at the evaluation and need not be faxed or emailed. Electronic pictures are welcome and appreciated.

3. Deposit:
A deposit of $300.00 is necessary to schedule and secure an appointment. We accept Cash, Checks, Visa, Master Card and Discover.

4. Scheduling an Appointment: Once we receive your completed History Form and deposit, we will contact you to schedule an appointment.

5. Appointment Day:

  • The evaluation process takes approximately 2 ½ hours; 1 hour for the assessment of the child, 30 to 45 minutes for the parent consultation and 30 minutes to 1 hour for equipping and rescheduling

  • Your evaluator will design an Individualized Neurodevelopmental Program (INP) specifically designed to meet your child’s needs, either the day of your appointment or within one week.

  • When your INP is ready, you will receive an email with login information so you can view online video clips of the activities on your child’s INP. You will have 30 days access to the online INP. High speed internet access is required to view these video clips. If you don’t have high speed internet access at home, please have an alternate plan such as going to your local library, using a spouses computer at work, taking a laptop to an establishment that offers internet access, or ask a neighbor, friend or family member if you can view your child’s program video clips at their home.

6. Schedule your re-evaluation:
Services and support are based on four month time periods, or trimesters. A re-evaluation every four months is necessary to update your child’s program and continue toward neurological efficiency. A deposit of $225.00 is due at each evaluation in the form of a post dated check or credit card information, which will be held until 5 weeks prior to your next appointment. Appointments must be changed or cancelled five weeks in advance in order to avoid forfeiting your deposit.

* ADDITIONAL FEES:
Many of the Neurodevelopmental materials you will need to implement your program can be handmade; however, we have found that the majority of our clients prefer the convenience of purchasing these materials ready-made. The cost of these materials is typically between $50.00 and $150.00 for the Initial Evaluation and less for Re-evaluations.

** TRIMESTER POLICY:
Services our offered on a trimester basis (4 months). Each trimester begins with an evaluation and includes a new INP (program), and four months of support. In order to receive continued support and to qualify for the discounted Re-evaluation rate of $475.00, you must be seen within 4 months of your last evaluation. In rare cases, the time between evaluations can be extended to 6 or 8 months, but this is not recommended as the INP needs to be updated frequently in order for it to be effective. An extension requires submitting a report and $50.00 fee, in order to keep your status as a current client and continue to receive support and the discounted re-evaluation rate during the extension period. If you go longer than 8 months between evaluations, the cost of the Re-evaluation goes back up to $625.00.
